The Hoosick Falls Health Center is a fully licensed 82 bed long term care facility certified to provide care under both Medicare and Medicaid programs. It is located at 21 Danforth Street in the Village of Hoosick Falls, New York. The Health Center is one of the few not-for-profit long term care facilities serving the residents of Rensselaer and Southeastern and central Washington Counties in New York, Southwestern Vermont and Northwestern Massachusetts. The Health Center has enjoyed consistent high ratings in the annual New York State Department of Health Surveys.
Every effort is made to avoid an atmosphere of "institutionalization" by creating a setting for each resident that maintains a feeling of home and family where "neighbors take care of neighbors". 
The Center has a Newly renovated and expanded Rehab Department with state of the art therapy equipment.
