Admission Policy

Hoosick Falls Health Center Admission Policy and Procedure

Policy: The Hoosick Falls Health Center shall admit and retain residents only on the recommendation of a licensed physician.

Purpose: The Hoosick Falls Health Center’s Admission Procedure conforms to the New York State Department of Health and Federal long-term care guidelines.

Procedure: Upon inquiry (re: admission), an application is mailed along with a medical records release form to the applicant or their designated representative. Submission of a completed application, however, does not imply or guarantee admission.

Criteria: The Hoosick Falls Health Center will admit and retain residents to our skilled nursing facility that meet the following criteria:

  1.  
    1. The resident must be qualified for residential skilled health care according to the Screen referral, and the resident must be in one of the Resource Utilization Groups, derived from the PRI assessment, which specifies the need for skilled nursing facility care,
    2. The facility’s ability to provide the necessary care required for the resident,
    3. Appropriateness of placement, i.e. Family/Resident acceptance of placement,
    4. Assignment and/or choice of a physician,
      * The resident may use the services of medical physicians engaged by Hoosick Falls Health Center, or may, at the resonates personal expense, retain his/her own physician, provided the latter (or his designee, in the absence of said physician) meets the requirement of the Health Center’s medical staff and agrees to abide by the rules and regulations of the Health Center’s medical staff, and also agrees to abide by all Federal and State laws and regulations regarding the provision of physician services to residents of long-term care facilities,
    5. Case mix need,
    6. Availability of male/female bed,
    7. The time frame with which a potential resident can be admitted once a bed is available,
    8. Admission application and medical release form on file,
    9. A Medical Doctor’s order is mandatory for admission and care,
    10. A durable power of attorney/conservatorship/health care proxy has been established and a copy of the appropriate document has been received with the application,
    11. Payment source has been established,
    12. A Tuberculosis Skin Test, know as PPD, performed and the results documented by a licensed physician,
    13. The Hoosick Falls Health Center will not admit:
      1. Residents under 16 years of age.
      2. Prenatal, intrapartum or postpartum maternity patients.

Determining Admissions:

  1.  
    1. Priority will be given to community residents that are awaiting admission and to those former residents that have been discharged from the Health Center and are again in need of Nursing Facility Car. All PRI/Screen referrals will be logged as received by the Resident and Family Services Department,
    2. The Admissions Committee (Administrator, Director of Nursing Services, and Resident and Family Services vices Staff) reviews the referral to determine the appropriateness of placement,
    3. * The PRI/Screen is reviewed by the Admissions Committee for:
      1) RUG’s classification,
      2) Payment source (Private, Medicaid, Medicare),
      3) Assess and evaluate candidate’s medical needs,
      4) Determine if candidate’s medical needs can be met with the facility’s case mix,
    4. Resident and Family Services contact referring agency/candidate/designated representative to determine interest in placement at the Health Center,
    5. Candidate and family member/or designated representative are personally interviewed by a member of the Admissions Committee, and a complete assessment of the candidate’s medical, emotional and physical needs is made,
    6. The candidate’s discharge potential is ascertained by the Resident and Family Services staff via contact with referring agency/licensed physician.
      * A physician must be established for the candidate prior to admission,
    7. Candidates for admission who meet admission criteria will be placed on the Hoosick Falls Health Center waiting list, which is updated weekly, and filed in the Resident and Family Services Office,
    8. Bed vacancy is determined by:
      1 ) Discharge of resident to another facility or home,
      2 ) Expiration,
      3 ) Termination of bed hold days,
    9. Prior to admission, the following information must accompany the resident:
      1 ) PRI and Screen,
      2 ) Physician’s Discharge Orders,
      3 ) Recent History and Physical,
      4 ) Discharge Summary,
      5 ) Lab Work,
      6 ) PPD Skin Test with Results,
    10. The policy of the Hoosick Falls Health Center is to admit residents and provide services without regard to race, creed, color, handicap, blindness, national origin, sexual preference, source of payment or sponsorship in admission, retention and care of residents,
