Philosophy and Mission Statement

The Health Center is committed to providing health care to persons in need of skilled services in an atmosphere of respect and concern for each individual. We are dedicated to meet the total emotional, physical and spiritual needs of its residents. We endeavor to accomplish our mission through a highly qualified and compassionate professional staff.

Our mission is to assist each resident to reach their maximum potential and be as independent as possible. The Hoosick Falls Health Center is committed to serving the needs of the community and will continue to meet the needs as they change.

The Hoosick Falls Health Center is a fully licensed 82 bed long term care facility certified to provide care under both Medicare and Medicaid programs. It is located at 21 Danforth Street in the Village of Hoosick Falls, New York. It is one of the few not-for-profit long term care facilities serving the residents of Rensselaer and Southeastern and central Washington Counties in New York, Southwestern Vermont and Northwestern Massachusetts. The Health Center has enjoyed consistent high ratings in the annual New York State Department of Health Surveys. The Hoosick Falls Health Center's ownership is vested in a volunteer 15 member Board of Directors.

There is a large dining room, resident activity areas, resident "personal care rooms" and several "quiet rooms" -- lounges for residents to meet with family, friends and clergy. An open patio provides residents with a secure place to get sun and fresh air in this pastoral setting.

A rehabilitative therapy room helps the short and long term care residents achieve as much independence as possible. Physical, Occupational and Speech Therapy are offered to our residents in this environment. There is a Recreational Therapy program that is designed to provide an ongoing program of meaningful recreational, social and spiritual activities to meet the interests and needs of the residents. It enables residents to engage in normal pursuits and promotes their physical, mental and social well-being and maintains their feelings of self-worth.

Other important services provided to our residents include: barbering and cosmetology, medical consultations, laboratory and radiology services, nutritional, dental and podiatry services. Also, there is a "state of the art" handicap accessible wheelchair lift van available to take residents to physician, eyecare, and other related appointments.

Every effort is made to avoid an atmosphere of "institutionalization." This is accomplished by creating a setting for each resident that maintains a feeling of home and family. Ultimately, this helps to produce emotional security and a sense of belonging, which in turn produces a healthy, satisfied resident.
